Terms of Registration
You must agree to the following terms when registering on our website.
Model release
I hereby grant permission for myself and my children to be photographed or video taped during Events and Youth Events with the understanding that I or my children can refuse to be photographed or videotaped at any time. Furthermore, I hereby grant permission for the photos to be edited and or published (church web site, bulletin, etc...) for the purpose of promoting Programs and Youth Programs at St. Elizabeth Ann Seton Catholic Church.
Emergency Information and Consent
All Participants should be covered by their own medical insurance policies. St. Elizabeth Ann Seton Parish has opted to pay the Archdiocese Catholic Mutual Group for Accidental Insurance Coverage for each Religious Education and Youth Ministry participant in the parish. This insurance covers all students while on church premises or on any field trip, retreat, or any church sponsored activity. This is supplementary to the regular insurance coverage, which has a limited excess medical coverage.
Emergency medical/dental release: I hereby give my consent for St. Elizabeth Ann Seton Catholic Church staff and/or volunteers to seek any emergency medical and/or dental aid deemed necessary for the protection and welfare of my child.
I give my child permission to attend Youth Ministry related activities on or away from campus with prior notice and registered drivers. I hold harmless St. Elizabeth Ann Seton Church and its agents, all persons chaperoning, driving or facilitating. I also agree that my child and I will be trained in the "Child Lures" Program which is required by the Archdiocese of San Antonio.

Discipline Policy
We feel an obligation to all children and teachers alike to maintain an orderly and harmonious classroom setting for the benefit of all. We rely on teachers and parents working together to establish an optimal learning environment. In the case that a child’s behavior becomes disruptive to the classroom environment the following policies will be implemented.
- Verbal correction from the teacher
- A gentle reminder to the child that they have been corrected once and yet their behavior is still keeping others from learning.
- 3rd correction warrants a visit to the Faith Formation Director.
- Director counsels the child about why their behavior is keeping others from learning and informs the child that their parents will need to be contacted if their disruptive conduct continues.
- Director contacts the parent giving them an option to:
- Accompany their child to class until the teacher, parent, and director feel the child is ready to comply.
- Or, Faith formation material are given to the parent for use at home for the remainder of the school year with the understanding that they can begin again in class the following year.
Cell phones and other electronics are considered a great disruption! Parents, please talk to your children about not using their phones during class.
Late Pick-Up Policy
There will be no supervision for children
who are not picked up within 10 minutes of class dismissal. Unfortunately, due to legal matters
concerning unsupervised minors, a child for whom this becomes an issue will no
longer be able to participate in our program.
Retreat Policies
- Sexual/Physical/Substance abuse must be reported by law to state or local law authority within 48 hours. Failure to report abuse of a minor could result in license suspension, 180 days imprisonment, &/or $2000.00 fine. Family Violence Hotline 1-800-252-5400
- No "Purple" –any intimate exchange on retreat or at meetings.
- No alcohol, drugs or tobacco use on retreat.
- No air horns or weapons
- Teens may not drive themselves to or from the day retreat/retreat…no exceptions.
- Any non-team member must have permission from the Adult Director to be on retreat premise.
- All retreat participants are expected to pay retreat fees to SEAS prior to the retreat.
- Any violation of the above policies will result in immediate removal from the retreat. Parents will be called and expected to promptly retrieve their child from the retreat center.
